How Do I Apply?

If you would like to apply for admission to the Upward Bound program, you should follow the following steps:

• Secure an application from your high school counselor or from the Upward Bound office by calling 740-368-3215.
  OR Download Application.
• Print or type all pages of the application.
• Obtain necessary signatures.
• Print or type the student essay.
• Ask your high school counselor to give you a copy of your transcript to attach to your application or have the counselor mail it directly to the program office.
• Ask your teacher(s) or counselor(s) to complete the recommendation form to attach to your application or they may mail it separately.
• Ask your parents to attach a copy of their completed federal tax form or income verification from the Department of Human Services.
• Return all parts to the Upward Bound office.
